Athletic Clearance Instructions

PLEASE NOTE STUDENTS MUST BE CLEARED TO PARTICIPATE IN ANY CONDITIONING AND/OR TRYOUTS!

To receive clearance, athletes must:
  1. Create a NEW Athletic Clearance account for the school year at athleticclearance.fhsaahome.org/register
  2. Compete all fields in the 'Student', 'Parent/Guardian', 'Medical', and 'Additional Questions' sections.
  3. Upload proof of medical insurance.
  4. Complete all student and parent/guardian signatures.
  5. Upload a valid EL2 physical form completed by a physician  Sport physicals are good for ONE calendar year https://fhsaa.com/documents/2026/2/25/EL2_022426.pdf
  6. Upload a current EL1 Electrocardiogram Screening form completed by a physician **Per the “Second Chance Act” (Senate Bill 1070), that went into effect on July 1, 2026** ECG/EKGs are good for TWO calendar years https://fhsaa.com/documents/2026/2/12/EL1_ECG_Screening_021126.pdf?path=enc
  7. Upload Certificates of Completion for each of the four NFHS Learn FREE courses below Certificates are good for ONE school year
  8. Upload EL1/2S - Medical Eligibility Supplemental Form
  9. Submit clearance for Athletic Director approval 
  10. Once you complete a clearance for one sport and arrive at the Confirmation Message, you will have the option to check off additional sports/activities for the current school year. 
  11. Watch your e-mail for approval/denial messages. If denied, you will see the reason(s) for denial listed in the e-mail. The student is not cleared until reciept of the approval e-mail.

PLEASE NOTE SCPA WILL NOT, UNDER ANY CIRCUMSTANCES, ACCEPT ANY HARD COPIES OF ANY OF THE ABOVE DOCUMENTS - ALL DOCUMENTS MUST BE UPLOADED THROUGH THE ATHLETIC CLEARANCE WEBSITE TO ENSURE CONFIDENTIALITY AND SECURE PROCESSING.
